BACKGROUND
The Non-Communicable Disease Control (NCDC) program of DGHS, MOHFW, initiated an NCD Management Model that recognizes the need for addressing NCDs among pregnant women and across the population. The model emphasizes screening for identification, managing, and following up on the “at-risk population”—pregnant women of any age and people aged over 40 years.
OBJECTIVES
Develop and demonstrate an automated NCD management model for identifying, screening, referring, and managing at-risk populations.
